A Look Inside The Your Health Matters Team
An exciting and rewarding opportunity has opened up within the Keswick and Solway Integrated Care Community (ICC) / Primary Care Network (PCN) for a Care Coordinator to join The Your Health Matters Team.
🎬 Watch the short video on this page as Jodie and Annarose from the team share more about what they do, the difference it makes, and why this approach to care is so important.
Working as part of the team means being at the heart of person-centred care, supporting patients, families, and other professionals to make sure people get the right help at the right time. The role involves working closely with GP practices and Primary Care Networks (PCNs) across the area to coordinate care and ensure a joined-up approach.
There are also opportunities for further professional development — including training routes to become an Assistant Practitioner or Nurse Associate for those who wish to build their career in healthcare.
"At the end of the day to know you've made a difference in someone's life is amazing."
Jodie Simpson
"I find it really rewarding because we're working with patients in the community to help them accomplish personalised goals. " Annarose Johnston
